Please make sure you read over it carefully, as there may be vital information contained within that can affect your treatment. This section covers everything about preparing for your procedure, as well as your post-operative instructions. SCHEDULING AND FINANCIAL
This section will help you navigate any questions that you may have about scheduling an appointment with us. You may reach out to us directly to schedule an appointment, or you can make an appointment using our convenient online appointment form. You may also schedule an appointment with us in person. We take our cancellation policy very seriously, as other patients are interested in seeing us, as well. Our no-show fee is explained in this section.
If you have any questions about insurance, please look over this section first. If you do not find the information you are looking for, we can help clarify any questions that you may have. We are providers for many different types of insurances, including but not limited to Aetna PPO/DMO, Cigna PPO/DHMO, Delta Dental (Premier, HMO/PPO), and MetLife. We also accept CareCredit® at our office.

YOUR FIRST VISIT
Your first appointment with us will allow us to get a better understanding of your dental concerns. Please understand that we may require multiple appointments to finalize your care and treatment plan. Before your appointment, we will ask you to have specific information ready for us, including your medical and health information, your medications, any referrals or X-rays you may have already, and your insurance information.
